About PBO: PBO offers free vision care services to uninsured and underserved children and adults in Ohio. Our goal is to reduce unnecessary vision loss and impairment by providing comprehensive vision care. We serve individuals who lack vision insurance or are underinsured and living at 200% poverty or worse.

Partner Organization's Responsibilities:

• Make referrals through PBO’s online portal.
• Act as a liaison between the client and PBO.
• Track client's usage of materials sent.
• Attend PBO webinars.
• Notify us of any staff changes within your organization.

Client Qualification: Clients must meet the most current 200% Federal Poverty Guidelines and lack vision insurance or be underinsured to qualify for our services.

Referral Process: To refer a client, complete an application for the appropriate eye care through our online portal. The application will be processed within 2 to 3 business days.

Client's Responsibilities:

• Provide accurate information on the application.
• Respect eye care professionals.
• Attend scheduled appointments.
• Behave appropriately.
